1. Fees – 

   1. Membership

      1. $20 per month OR $200 per year for initial person;

      2. $5 per month OR $50 per year for additional family member;

   2. Key Fob/Card – initial fee of $20 for purchase of card

      1. Each card will be signified with MC2 Logo Information AND Serial Number;

      2. Replacement cards will be provided after additional fee has been paid;

      3. All Fob/Card uses will be recorded via use and checked weekly – any unauthorized use will be addressed immediately;

      4. Fob/Cards should ONLY BE USED by individual that is/was issued the specific FOB/CARD – any unauthorized use of FOBs/CARDs will be addressed immediately and could result in suspension/termination;

      5. If/when fees are not paid, FOB/CARD will be terminated until paid;

   3. Funds – 

      1. Membership funds will be deposited into a specific Student Activities account and used specifically for Community Use of Weight-Training room needs

      2. Key Fob/Card funds will be specifically for the purchase of the Key Fobs/Cards

   4. Payments will be collected by the CENTRAL OFFICE

2. Available Times – 

   1. Monday – Friday – 5 a.m. to 7 a.m.

   2. Monday – Friday – 6 p.m. to 9 p.m.

   3. Saturday – 5 a.m. to 10 p.m.

   4. Sunday – 12 p.m. – 10 p.m.

   5. ***Room will NOT be available for Community Use during Student Use activities/practices/workouts***

   6. ***Room will NOT be available for Community Use during Home Basketball Events***

3. Community Use is only for NON-STUDENTS or ADULTS – no minor shall be using the Weight-Training Room without presence of Staff Member OR Parent-Guardian

4. No One shall use WEIGHT-TRAINING ROOM without fully completing the USER AGREEMENT

5. Milan C-2 School District reserves the right to suspend or terminate membership with or without refund of FEES

6. Milan C-2 School District will assign one (1) staff member to serve as the Weight-Training Room Community Coordinator (WTRCC) – WTRCC will:

   1. Keep organized records of members;

   2. Review the facility EVERY DAY 

   3. Communicate issues with MEMBERS or appropriate School Staff, depending on need

7. Milan C-2 School District is not responsible for lost/damaged items or any illness/injury suffered during use of the WEIGHT-TRAINING ROOM

8. Door leading to MAIN School Buildings will be LOCKED and only staff with keys can enter campus – NO Key Card/Fob will allow entry to main campus

9. Single bathroom and drinking fountain are both available within room

10. Non-secured storage containers will be placed in room – for storage DURING times when attendees are using facility; again MC2 is NOT responsible for lost or stolen items;

11. Room will be OPENED for Community Use on/around May 2nd, 2022;

12. Milan C-2 School District reserves the right to close or amend the schedule of Community USE if/when needed for school sponsored/sanctioned activities;

13. Weight-Training room contains Security cameras;

***COMMUNITY USE IS ONLY FOR NON-STUDENTS OR ADULTS – NO MINOR SHALL BE USING THE WT ROOM WITHOUT THE PRESENCE OF A STAFF MEMBER OR THE STUDENT’S PARENT OR GUARDIAN***

***NO ONE SHALL USE THE WT ROOM WITHOUT FULLY COMPLETING AND SUBMITTING THE USER AGREEMENT FORM***

***NO ACCESS TO THE SCHOOL CAMPUS WILL BE PERMITTED BY USERS OF THE WT ROOM***

***MEMBERS OF THE MC2 WT ROOM WILL BE EXPECTED TO KEEP AREA CLEAN AND ORDERLY***

***NON-SECURED STORAGE CONTAINERS WILL BE AVAILABLE DURING WT ROOM USE***

***WATER FOUNTAIN AND SINGLE-USE RESTROOM ARE AVAILABLE***

THE MILAN C-2 SCHOOL DISTRICT RESERVES THE RIGHT TO SUSPEND OR TERMINATE MEMBERSHIP WITH OR WITHOUT THE REFUND OF FEES CAMERAS ARE PRESENT IN WT ROOM

THE MILAN C-2 SCHOOL DISTRICT IS NOT RESPONSIBLE FOR LOST/DAMAGED ITEMS OR ANY ILLNESS/INJURY SUFFERED DURING THE USE OF THE WT ROOM OR EQUIPMENT – USE AT YOUR OWN RISK
